Fill out the "Order/Renew Your Kit" form and reserve
your pickup slot.

Please do not visit the London Chamber of Commerce until your
scheduled pickup time.

Each company should assign a Screening Supervisor Designate who
will be the contact person.

If you have multiple businesses/locations, please fill out one per business/location.
